Use this form to request an exam for your pet.  Once you submit your request will go into the queue and our schedulers will book you into the first available appointment.  Please note, payment for the appointment is due at the time it is scheduled and if you no show you forfeit your entire deposit.  You will receive both text and email communications from us with the appointment date, time, and any further instructions.
